If both pet parents agree that Pet Court is needed to determine who gets custody of pets, this Intake form must be completed by each parent, separately, and submitted by clicking the blue “SUBMIT” button at the bottom of this page. After receiving the Intake from each parent, directions for signing the Contract and paying Pet Court will be provided by DocuSign, an online contract management service.
